Require the Department of Human Services to procure a master contractor to implement and operate a transitions clinic network pilot program for previously incarcerated individuals with chronic health conditions.

Master contractor must provide continuity of care, comprehensive primary care, and patient-centered medical services through at least one primary care health clinic.

Master contractor shall employ two community health workers, partner with local reentry organizations, and integrate behavioral health services and medication-assisted treatment for substance use disorders.

Master contractor must submit quarterly reports to the House and Senate committees on housing and human services and may retain an administrative fee not exceeding 15% of the total contract price.

Appropriate general revenues for fiscal year 2020-2021 to the Department of Human Services for procuring the master contractor and implementing the pilot program, effective July 1, 2050.
