Establishes a statewide transition success network for high school students with disabilities in Hawaii public schools and public charter schools, effective July 1, 2050.

Requires the Department of Education superintendent to appoint a statewide transition coordinator by October 1, 2021, and designate an interagency planning group as the statewide advisory council by January 1, 2022.

Mandates each school district establish a transition center by school year 2021-2022 with a district coordinator to oversee high school transition plans and manage advisory councils with representatives from state agencies, employers, families, and community organizations.

Requires each high school to submit annual transition plans by March 31, 2022, detailing services and support for students with disabilities including job training, internships, self-determination, independent living skills, benefits planning, and coordination with state agencies.

Allocates 10 percent of appropriated funds to the University of Hawaii Center on Disability Studies for job coaching and intermediary services, allows superintendent to reserve up to 5 percent for implementation, and distributes remaining funds to districts proportionally based on number of high school students with disabilities.
