S.C.R. No. 211 Summary

• Office of Primary Care and Rural Health is requested to facilitate discussions with key community health and social service organizations in Hawaii County to address health and wellness needs of vulnerable populations.

• The goal is to align objectives of multiple organizations into a single shared "2030 One Shared Health Vision Plan" for Hawaii County that addresses preventative care, primary care, specialty care, and emergency care.

• The plan should examine social determinants of health and establish a collaborative health network model for Hawaii County that could serve as an example for other counties.

• Office of Primary Care and Rural Health must submit the completed 2030 One Shared Health Vision Plan to the Legislature no later than twenty days before the convening of the 2021 Regular Session, including any proposed long-term legislative grant-in-aid requests.

• Certified copies of the resolution are to be transmitted to the Director of Health and the Mayor of Hawaii County.
