Office of Primary Care and Rural Health is requested to facilitate discussions with key community health and social service organizations in Hawaii County to address health and wellness needs of vulnerable populations.

The goal is to align the goals and objectives of multiple organizations into a single 2030 One Shared Health Vision Plan for Hawaii County.

The plan should examine social determinants of health and coordinate efforts across preventative care, primary care, specialty care, and emergency care services.

Office of Primary Care and Rural Health must submit the completed 2030 One Shared Health Vision Plan, including any proposed long-term legislative grant-in-aid requests, to the Legislature no later than twenty days before the start of the 2021 Regular Session.

Certified copies of the resolution must be transmitted to the Director of Health and the Mayor of the County of Hawaii.
