• Requires major social service providers with assets exceeding a specified threshold to coordinate with the state-designated health information exchange entity to share data on social determinants of health, including food, nutrition, transportation, housing, and personal care services.

• Makes enrollment in the Hawaii immunization registry mandatory for all healthcare providers administering immunizations, including COVID-19 vaccines, effective August 1, 2021, unless patients or their guardians opt out.

• Directs the examiner of drivers to coordinate with the state-designated health information exchange entity to link organ donor information from driver's licenses to the statewide health information exchange.

• Requires the department of health to coordinate with the state-designated health information exchange entity to link Hawaii immunization registry information to the statewide health information exchange.

• Expands the definition of "health care provider" to explicitly include providers of COVID-19 vaccines and adds "COVID-19 vaccine" as a defined term covering vaccines against SARS-CoV-2 and its mutations.
