Directs the Department of Health to establish and administer a wholesale prescription drug importation program that meets federal requirements under 21 U.S.C. section 384, including safety and cost savings standards.

Requires the program to import prescription drugs from Canadian suppliers regulated under Canadian law, implement drug purity testing, and restrict distribution to Hawaii only while limiting pharmacy markups to actual acquisition costs.

Mandates participating health plans update formularies, base patient cost-sharing on actual acquisition costs, and demonstrate that savings are reflected in premium reductions to the satisfaction of the director of health.

Requires the Department of Health to submit a federal certification request to the U.S. Department of Health and Human Services and begin operations within six months of approval, including becoming licensed as a wholesaler and establishing a publicly available price list.

Requires annual reports to the governor and legislature detailing covered drugs, participating entities, prescriptions dispensed, estimated consumer savings, and audit findings.
