Requests physicians and surgeons to determine post-fertilization age of unborn children before performing abortions and to decline abortions on unborn children at 20 weeks or older unless exceptional circumstances exist.

Defines exceptional circumstances to include: (1) abortion necessary to save the pregnant woman's life from physical disorder or illness; (2) pregnancy resulting from rape with documented counseling or medical treatment obtained at least 48 hours prior; (3) rape reported to law enforcement; or (4) rape or incest against a minor reported to law enforcement or child abuse agency.

Requires physicians to provide pregnant women with a disclosure form indicating the probable post-fertilization age and potential fetal pain capability, with signatures from the woman, physician, and a witness before proceeding with abortion.

Requests physicians to decline performing abortions by dismemberment except when necessary to save the pregnant woman's life from physical medical conditions.

Requires physicians to submit annual summaries to the Department of Health detailing abortions performed on unborn children at 20 weeks or older, including post-fertilization age, method, location, exceptional circumstance cited, and any live birth incidents.
