Establishes a new chapter requiring the governor to designate a state entity to develop and maintain a secure, interoperable health information exchange database containing medical records of patients receiving treatment at health care and wellness service providers in Hawaii.

Mandates all health care and wellness service providers operating in the State submit complete and accurate patient health records to the state designated entity for inclusion in the health information exchange.

Requires medical records comply with HIPAA (Health Insurance Portability and Accountability Act of 1996), the 21st Century Cures Act of 2016, and related federal regulations regarding protected health information.

Allows health care and wellness service providers addressing social determinants of health to make collected information available to the state designated entity, and permits providers to use the exchange to establish patient portals for accessing protected health information.

Takes effect July 1, 2060.
