Requires all underground injection wells in Hawaii to obtain a National Pollutant Discharge Elimination System (NPDES) permit from the Director of Health by January 1, 2031.

Prohibits underground injection without an NPDES permit beginning January 1, 2031.

Maintains current restrictions that the Director cannot issue permits for sewage wastewater injection wells unless alternative disposal options are not available, feasible, or practical.

Applies the decision in County of Maui v. Hawaii Wildlife Fund (2020) to address downstream impacts of injection wells on coastal ecology, coral reefs, human health, and recreation statewide.

Establishes a 10-year transition period for existing injection wells to comply with the new permitting requirement.
