Establishes an Office of Health Equity within the Department of Health to advance health equity by addressing social determinants of health through multi-sector collaboration.

The office shall assess and monitor population health disparities, identify and engage stakeholders on policy and environmental improvements, evaluate efforts using short- and long-term measures, and reassess strategies.

Authorizes the office to appoint two advisory groups at the director's discretion: a policy advisory group (up to 18 members) focused on racial, ethnic, gender, sexual minority, disability, and geographic health equity; and a community network advisory group to identify community needs and recommend solutions.

Appropriates unspecified general revenue funds for fiscal year 2022-2023 to establish and staff the office and hire contractors.

Effective date is January 1, 2050.
