Directs the Office of Primary Care and Rural Health to initiate a "2030 One Shared Health Vision Plan" with a pilot program in Hawaii County.

Requires the Office of Primary Care and Rural Health to facilitate discussions with key community health and social service organizations in Hawaii County to address health and wellness needs of vulnerable populations by aligning organizational goals and objectives.

Mandates submission of a report on the 2030 One Shared Health Vision Plan to the legislature no later than twenty days prior to the 2023 regular session, including any proposed long-term legislative grant-in-aid requests.

Takes effect on January 1, 2050.
