• Requests the Office of Primary Care and Rural Health to establish a 2030 One Shared Health Vision Plan and pilot program in Hawaii County to address rural health care delivery challenges.

• Directs the office to facilitate discussions with community health and social service organizations in Hawaii County to align goals, objectives, and services for vulnerable populations.

• Hawaii County ranks third nationally for primary health care worker shortages with 53% fewer doctors than comparable mainland counties due to provider retirements, relocations, and reduced hours.

• Requires submission of a report on the 2030 One Shared Health Vision Plan, including proposed long-term legislative grant-in-aid requests, no later than 20 days before the 2023 regular legislative session.

• Directs certified copies be transmitted to the Director of Health, Chief of the Family Health Services Division, and Mayor of Hawaii County.
