Request the Office of Primary Care and Rural Health to initiate a 2030 One Shared Health Vision Plan and establish a pilot program in Hawai'i County to address the county's disproportionate health care challenges.

Require the Office to facilitate discussions with key community health and social service organizations in Hawai'i County to align goals, objectives, and service availability to address health and wellness needs of vulnerable populations.

Direct the Office to submit a report on the 2030 One Shared Health Vision Plan to the Legislature no later than twenty days prior to the convening of the 2023 Regular Session, including any proposed long-term legislative grant-in-aid requests.

Address Hawai'i County's documented primary health care worker shortage, with the county ranking third nationally with fifty-three percent fewer doctors than comparable mainland counties due to provider retirements and relocations.

Transmit certified copies of the resolution to the Director of Health, Chief of the Family Health Services Division, and Mayor of Hawai'i County.
