Creates a medical release program allowing the Hawaii Department of Public Safety to assess and refer eligible inmates to the Hawaii Paroling Authority for possible early release based on medical grounds.

Establishes four criteria for medical release eligibility: terminal illness with poor prognosis, seriously debilitating irreversible mental or physical condition, cognitive or physical incapacity to participate in rehabilitation, or medical condition requiring care complexity the department cannot provide long-term.

Requires inmates pose a low risk to public safety and allows requests to be initiated by the director, inmate, or inmate's representative with physician recommendation; director may deny frivolous or clearly ineligible requests before forwarding to paroling authority.

Mandates Hawaii Paroling Authority conduct hearings on all medical release requests and prohibits release of inmates who pose danger to society; denials do not affect eligibility for other parole or release forms unless medical condition changes.

Establishes conditions for released inmates including supervision by paroling authority, department home visits, and compliance with release conditions through sentence expiration; allows revocation and return to custody for condition violations with credit given only for compliant release period.
