Requires commercial agricultural entities that annually purchase or use more than a specified threshold of restricted use pesticides to disclose all pesticide use (restricted use, general use, and experimental use) in annual public reports.

Reports must be submitted to the Department of Health no later than 60 days after each calendar year ends, covering all outdoor pesticide applications from the prior year.

Report contents must include pesticide listings by federal and state registration numbers and active ingredients, total quantities used, and geographic location identification by tax map key number.

Reports are classified as public records and must be posted on the Department of Health website.

Department of Health shall adopt rules to implement the disclosure requirements, effective one year after the bill's enactment.
