• Department of Taxation is requested to create a simplified N-11 form for individuals without taxable income subject to state income tax

• Form should be as short as possible, requesting only necessary information, and provided in paper format only

• Simplified form is intended to help individuals reliant on social security or public assistance access unclaimed state tax credits and rebates, including the refundable food/excise tax credit

• Form must be available for use for taxable years beginning after December 31, 2022

• Certified copies of the resolution are to be transmitted to the Governor and Director of Taxation
