Eliminates Medicare Part B premium reimbursements for spouses of employees hired on or after July 1, 2023, while maintaining reimbursements for spouses of employees hired before that date.

State and county governments will no longer contribute to Medicare Part B premiums for newly hired employees' spouses, estimated to reduce future annual required contributions by $1.2 billion over a 30-year period.

Does not affect current retirees, vested terminated employees, or current employees hired before July 1, 2023.

Amends Hawaii Revised Statutes Section 87A-23 to restrict spousal Medicare Part B premium reimbursement eligibility based on employee hire date.

Takes effect January 1, 2050.
