Requires offerors to submit the standard form questionnaire on an annual basis to be prequalified as "responsible" for projects by the procurement office.

Changes information submitted on the questionnaire from confidential (available only to law enforcement) to public information available for inspection and duplication under chapter 92F.

Expands the definition of "past performance" to include State, federal, and private contracts as factors in responsibility determinations for capital improvement projects.
