Requires the State Council on Developmental Disabilities to submit a report to the legislature no later than twenty days prior to the 2025 regular session on health disparities experienced by persons in Hawaii with developmental or intellectual disabilities.

Report must include definitions of the state's developmental/intellectual disability population, social determinants of health, and population-level differences in health indicators between disabled and non-disabled individuals.

Report must address household income, employment status, education level, access to health information technology, transportation, prevalence of disabilities by zip code, ethnicity breakdown by zip code, and health outcome comparisons.

Report must include recommendations for proposed legislation and best practices to reduce health disparities for persons with developmental or intellectual disabilities in Hawaii.

Appropriates unspecified funds from general revenues for fiscal years 2023-2024 and 2024-2025 to the Department of Health to compile and submit the report; effective date December 31, 2050.
