Continues Medicare Part B premium reimbursement for spouses of employees hired before July 1, 2023, and for current retirees, vested terminated employees, and current employees.

Estimates $1.2 billion reduction in future state annual required contributions over a 30-year period through elimination of spousal Medicare Part B premium reimbursement for new hires.

Amends section 87A-23 of Hawaii Revised Statutes to limit spousal reimbursement eligibility based on employee hire date.

Takes effect upon approval and does not affect rights, duties, or proceedings that matured before the effective date.
