Authorizes the issuance of special number plates that commemorate Duke Kahanamoku and recognize his contributions to Hawaii as a waterman, Olympic medalist, lifeguard, and ambassador of aloha.

Requires the director of finance of Honolulu to establish a special plate design in consultation with other county finance directors, police chiefs, and the Outrigger Duke Kahanamoku Foundation, with the design containing words or images recognizing Duke Kahanamoku.

Requires written permission from Malama Pono, Inc. (owner of the Duke Kahanamoku trademark) before issuance, and mandates that all proceeds benefit the Outrigger Duke Kahanamoku Foundation.

Charges a special number plate fee covering the county's costs plus a fundraising fee determined by the director of finance, with revenue deposited into a separate account to fund water safety and swim education programs statewide.

Allows the director to revoke all special plates if fewer than 150 registered owners obtain them within three years of the first issuance; effective July 1, 2050.
