Requires individual and group health insurance policies issued or renewed after December 31, 2024, to cover standard fertility preservation services for individuals undergoing medically necessary treatments that may cause infertility.

Applies coverage mandate to insurers, mutual benefit societies, and health maintenance organizations in Hawaii.

Prohibits insurers from using prior fertility diagnoses or treatments as grounds to exclude or restrict coverage, and prohibits discrimination based on life expectancy, disability, medical dependency, or perceived quality of life.

Defines standard fertility preservation services according to American Society of Clinical Oncology guidelines, excluding experimental procedures, third-party assisted reproduction technologies, and cryopreservation storage services.

Effective date is December 31, 2050.
