Exempts persons holding valid certifications as doulas or lactation specialists from nationally recognized organizations approved by the department from Hawaii's midwifery licensure requirements.

Expands the family care exemption under midwifery laws to include grandchildren, in addition to existing exemptions for spouses, domestic partners, parents, siblings, and children.

Establishes a Birth Attendant Working Group chaired by the attorney general to develop a regulatory structure for birth attendants, including fees, licensing requirements, renewal procedures, and grounds for license denial or revocation.

Requires the Legislative Reference Bureau to conduct a study on birth attendant regulation, including compilation of other states' laws, examination of regulatory frameworks, literature review of best practices, and recommendations for Hawaii regulation.

Directs both the working group and legislative reference bureau to submit reports with findings and proposed legislation to the legislature before the 2025 regular session; working group dissolves June 30, 2025.
