Authorizes the Department of Health to certify community care foster family homes for a fourth adult who is a Medicaid recipient with documented housing instability, subject to specified conditions including one year of certified operation and employment of certified nurse aides.

Requires the primary caregiver to be a certified nurse aide and a second caregiver (also a certified nurse aide) to be on duty from 7:00 a.m. to 7:00 p.m.; all substitute caregivers must also be certified nurse aides.

Mandates that a caregiver may only request to increase bed capacity from three to four after all vacancies in community care foster family homes on the same island are filled.

Requires the Department of Health and Governor's Coordinator on Homelessness to enter into a memorandum of agreement before placement of any fourth adult and to submit a report to the Legislature by twenty days prior to the 2025 regular session.

Takes effect July 1, 3000 and repeals on June 30, 2027; voids if conflict with federal funding requirements is found.
