Allows adults and parents/guardians of minors to change sex designation on Hawaii birth certificates to male, female, or "X" (non-binary option), effective January 1, 2024.

Requires applicants to submit a completed application with specific information including full name, date of birth, place of birth, parents' names, contact information, and requested sex designation.

Requires adults to notarize their application and sign under penalty of perjury; minors' applications must include a signed statement from a licensed health care provider attesting the minor is under their care and the request is consistent with the minor's identity.

Allows applicants unable to provide required information to submit a written explanation to the state registrar, who may approve the change if circumstances prevent the applicant from knowing required items.

Directs the Department of Health to create and make available necessary forms for all birth certificate amendment categories, effective July 1, 2023.
