Allows providers to object to claim denials (other than fee schedule disputes) within 90 days, triggering employer obligation to file with appeals board within 60 days; if employer prevails, appeals board may order provider reimbursement for unreasonable charges.

Requires written notification to providers, employees, and attorneys if the insurer or employer contests the reasonableness or necessity of medical expenses, with reasons provided.
