Requires all new birth certificates issued by Hawaii after this Act's effective date to include a sex designation based on the genitalia of the person born.

Allows amendments to sex designation on existing birth certificates for individuals 18 years or older with a physician or physician assistant affidavit confirming the designation is based on the genitalia of the birth registrant.

Eliminates previous requirements that individuals have "appropriate clinical treatment for gender transition" and have "completed the transition" to change sex designation on birth certificates.

Requires that original certificates amended under the sex designation provision be unsealed and marked as amended, unlike other amendments which remain sealed and unmarked.

Takes effect upon approval.
