H.B. 641 Summary

• Requires health insurers and mutual benefit societies to cover traditional native Hawaiian healing and cultural practices (including ho'oponopono, lomilomi massage, ai pono, and la'au lapa'au) when provided at federally qualified health centers or native Hawaiian health care systems, effective January 1, 2026.

• Coverage must apply to policyholders and their dependents with copayment, deductible, and coinsurance provisions no less favorable than other covered medical services.

• Insurers must reimburse providers offering these services and cover out-of-network providers without cost-sharing if no adequate in-network provider exists or if an in-network provider cannot provide services in a timely manner.

• Every insurer must provide written notice to policyholders about this coverage by December 31, 2026, with notice included in annual information mailings or other correspondence.

• Does not apply to limited benefit health insurance or services provided outside of federally qualified health centers or native Hawaiian health care systems.
