Allows physicians to practice at organized ambulatory health care facilities without requiring hospital privileges at a licensed hospital in the same geographic location as the facility.

Applies only to counties with populations of less than 500,000 (Hawaii's neighbor islands and rural areas).

Defines "organized ambulatory health care facility" according to section 323D-2 of Hawaii Revised Statutes.

Requires the Department of Health to evaluate the impacts on local health care systems, surgical access, financial stability, health care access, quality of care, and costs, with a report due by January 19, 2027.

Sunsets on June 30, 2028; takes effect on July 1, 2025.
