• Requires all health insurers in Hawaii, including Medicaid managed care programs, to cover continuous glucose monitors (CGMs) and related supplies for individuals diagnosed with diabetes, including gestational diabetes, regardless of insulin use.

• Coverage applies to policies, contracts, plans, or agreements issued or renewed after December 31, 2025, and includes costs for necessary repairs or replacement parts for CGMs.

• Defines a continuous glucose monitor as a device that continuously measures glucose levels through a skin sensor remaining in place for a minimum of seven days and transmits glucose data in real-time or at set intervals.

• Coverage is subject to the insured's existing copayment, deductible, and coinsurance provisions and requires that the device be medically necessary and prescribed by an authorized health care professional.

• Directs the Department of Human Services to seek federal authorization and waivers to implement the requirement under Medicaid managed care programs, effective July 1, 2025.
