Requests the Department of Agriculture to add a section to the State of Hawaii Plant and Animal Declaration Form requiring passengers, officers, and crew members to acknowledge awareness of federal, state, and county laws protecting public monuments, Native Hawaiian cultural sites, burial grounds, and places of worship.

Requires individuals completing the form to affirm awareness of federal and state laws protecting indigenous, endangered, and threatened wildlife, and to commit to following those laws.

Directs the Department of Agriculture to include information about how visitors can learn more about desecration and wildlife protection laws.

Specifies that once the Akamai Arrival Program (digital system) is fully implemented, the department should facilitate these disclosures through that program instead.

Requires certified copies of the resolution to be sent to the National Park Service Regional Director, Governor, Board of Agriculture Chair, Hawaii Tourism Authority leadership, and county mayors.
