Requests the Hawaii Employer-Union Health Benefits Trust Fund Board of Trustees and Med-QUEST Division to ensure 12 percent of monthly health insurance premiums are paid directly to each member-patient's primary care provider.

Requires the 12 percent premium payments to be made to primary care providers regardless of whether the member-patient receives actual services from that provider.

Identifies primary care physician shortage in Hawaii as partly resulting from high cost of living and business costs, with direct premium funding intended to help providers remain viable.

Directs certified copies of the resolution to be transmitted to the Governor, Director of Human Services, Med-QUEST Division Administrator, and Chair of the Hawaii Employer-Union Health Benefits Trust Fund Board of Trustees.
