Requests the Department of Education and Department of Health to update vaccination forms and recordkeeping procedures for public school and charter school students.

Addresses the current practice where enrollment coordinators designate students as "unvaccinated" if they claim an exemption for any single vaccine, regardless of their vaccination status for other vaccines.

Aims to create more accurate assessment of student vaccination statuses to enable the Department of Education to take proactive measures during disease outbreaks and determine whether herd immunity can be achieved in educational environments.

Requires certified copies of the resolution to be transmitted to the Superintendent of Education and Director of Health.
