• Request Department of Human Services and Department of Health to establish a program that incentivizes Community Care Foster Family Homes (CCFFH) to accept individuals eligible for the State's Medicaid I/DD waiver program

• CCFFH provides 24-hour living accommodations, personal care, and homemaker services as a more affordable alternative to institutionalized care facilities

• Address insufficient number of CCFFHs in the State currently accepting individuals eligible for assistance under the Medicaid I/DD waiver program

• Transmit certified copies of the resolution to the Director of Human Services and Director of Health
