Amends proxy form requirements for planned community associations (HRS 421J-4), cooperative housing corporations (HRS 421I-4), and condominium associations (HRS 514B-123) to include standardized explanatory language for each voting option.

Requires proxy forms to include detailed explanations of four checkbox options: quorum purposes only, designating a named individual, giving proxy to the board as a whole, or sharing the vote equally among directors present at the meeting.

Adds explanatory text clarifying that "quorum purposes only" counts toward meeting requirements but carries no voting power, and that personal attendance voids any previously submitted proxy.

Specifies that proxies returned with no box checked or with multiple boxes checked will automatically count for quorum purposes only.

Takes effect upon approval and does not affect rights, duties, penalties, or proceedings that existed before the effective date.
