Requires all health insurance policies, mutual benefit society contracts, and health maintenance organization plans issued or renewed after December 31, 2026, to cover standard fertility preservation services for individuals undergoing treatments that may cause iatrogenic infertility (infertility caused by medical treatments like chemotherapy or radiation).

Coverage applies to cancer-related medical treatments with likely infertility side effects, and insurers cannot deny coverage based on prior diagnosis, prior fertility treatment, life expectancy, disability, or perceived quality of life.

Standard fertility preservation services must follow the 2018 American Society of Clinical Oncology guidelines, which define established medical practices for fertility preservation in cancer patients.

Coverage excludes experimental procedures, third-party assisted reproduction (donor eggs, surrogates, gestational carriers), and cryopreservation storage services.

The bill amends Hawaii Revised Statutes chapters 431, 432, and 432D to apply the mandate uniformly across insurers, mutual benefit societies, and HMOs, with an effective date of July 1, 3000 (placeholder date).
