Establishes a Maternal Health Monitoring Pilot Program within the Department of Health to provide remote patient monitoring for pregnant Medicaid recipients with hypertension or diabetes during pregnancy and up to three months postpartum.

Targets up to 300 eligible participants who are pregnant, receiving medical assistance, and members of a participating managed care organization, using FDA-authorized devices to monitor blood pressure, weight, blood glucose, and other health data.

Requires technology vendors to deliver monitoring devices, provide training, employ a clinical care team of nurses and dietitians, and establish escalation pathways when concerning readings require additional medical attention.

Allocates $600,000 to offset pilot program costs, with contracts to be executed by June 30, 2027, and the program concluding two years after implementation or by December 27, 2029.

Mandates a report 24 months after implementation evaluating maternal, fetal, and neonatal health outcomes including mortality rates, preeclampsia, cesarean section rates, preterm birth, and potential Medicaid savings.
