Establishes "Hawaii Care," a universal single-payer health care system to replace all existing health care plans in the state and provide comprehensive coverage to all Hawaii residents.

Designates the Hawaii Health Authority as the responsible agency for planning, implementing, and administering the new system, including developing eligibility criteria, financing requirements, and budget projections.

Requires the Hawaii Health Authority to submit a comprehensive implementation plan to the legislature that includes cost projections, a benefits package, five-year fiscal projections with 95% actuarial value, and proposed funding mechanisms including potential income tax changes.

Mandates research into health care provider burnout, current insurer compensation practices, and other financial practices to prepare for the transition to Hawaii Care.

Requires federal waivers for Medicare, Medicaid, and the Hawaii Prepaid Health Care Act before replacing existing health plans, with the Authority authorized to contract with public or private entities for claims administration and quality assurance.
