Prohibits Hawaii's Department of Human Services from contracting with managed care organizations or other financial risk-bearing entities for Medicaid administration, with all existing managed care contracts terminating by December 31, 2026

Transitions Medicaid to a managed fee-for-service model where providers are paid directly by the state, with physicians receiving at least 100% of Medicare rates and care coordination funded separately through fixed monthly payments to designated primary care practices

Requires the Department to contract with administrative services organizations to perform non-risk functions including claims processing, prior authorization, provider credentialing, customer service, and data analytics

Establishes regional health hubs in each county with quarterly meetings to monitor community health needs, assess disparities, and facilitate feedback between providers, patients, and the Department

Creates a Medicaid Care Coordination Program contracting with community-based teams to provide patient navigation, chronic disease management, behavioral health integration, and culturally competent outreach services
