Prohibits healthcare providers from performing surgeries, prescribing puberty blockers, or administering hormones to minors for the purpose of enabling gender transition or treating gender dysphoria.

Applies to procedures performed in Hawaii or administered via telehealth to minors located in the state.

Provides exceptions for treating congenital defects, precocious puberty, disease, or physical injury, and allows ongoing treatments started before the Act's effective date to conclude by January 1, 2027, with physician certification.

Creates a private right of action allowing minors or their parents to sue for compensatory damages, punitive damages, and attorney's fees, with a 30-year statute of limitations from when the minor turns 18.

Authorizes the attorney general to bring civil actions with penalties of $25,000 per violation and requires licensing authorities to take emergency action against violating healthcare providers.
