Mandates all health insurers in Hawaii, including Medicaid managed care programs, to cover continuous glucose monitors (CGMs) and related supplies for individuals diagnosed with diabetes, including gestational diabetes, regardless of insulin treatment status

Applies to insurance policies, contracts, plans, or agreements issued or renewed after December 31, 2025, with the Act taking effect July 1, 2025

Coverage must include necessary repairs and replacement parts, with copayments, deductibles, and coinsurance no less favorable than those for other medical services and supplies

Directs the Department of Human Services to seek federal authorization and waivers as necessary to implement Medicaid provisions, with that section taking effect upon federal CMS approval

Authorizes the Department of Health and Department of Human Services to accept gifts, grants, and donations to support expanded CGM access, provided conditions are consistent with state law
