Establishes the Kalaupapa State Historical Area to preserve the Kalaupapa Settlement, educate the public, and commemorate the lives of Hansen's disease patients and those who served them

Transfers the county of Kalawao (Kalaupapa Settlement on Molokai) to become part of the county of Maui upon one year after the passing of the last patient resident, as currently only 7 individuals remain on the registry with 4 full-time residents, all age 83 or older

Repeals numerous sections of Chapter 326 governing Kalawao county operations, including provisions for the county sheriff, patient employment and pensions, the Kalaupapa store, fishing exemptions, and residency policies, effective one year after the last patient's passing

Requires the Department of Health to continue providing statewide services for Hansen's disease patients and mandates annual reporting to the legislature until the last patient passes or the Settlement is no longer under DOH jurisdiction

Mandates that future planning for transferring Kalaupapa Settlement to other entities must include Ka Ohana O Kalaupapa (designated in federal law) and a topside community organization selected in consultation with the Molokai community
