Establishes a three-year Health Coverage Continuity Pilot Program administered by the Department of Human Services to subsidize health insurance deductibles up to $1,000 per individual per year for preventive services and screenings.

Targets individuals who have lost Medicaid coverage and do not qualify for employer-sponsored insurance or federal premium subsidies, including seasonal workers, independent contractors, underemployed adults, and college students.

Eligible participants must be enrolled in a bronze level or catastrophic health insurance plan approved by the insurance commissioner.

Authorizes the Department to contract with nonprofit health insurers or community-based organizations to operate the program, with access to state-backed reinsurance and excess loss protection.

Requires annual reports to the legislature on enrollment, health outcomes, and fiscal impact; the program takes effect July 1, 2050, and repeals June 30, 2029.
