Establishes a Medicaid buy-in program for workers with disabilities in Hawaii under federal Social Security Act section 1902(a)(10)(A)(ii)(XIII).

Eligible individuals must have net countable income greater than 250% of the federal poverty level and be disabled under Social Security Act definitions, regardless of their ability to engage in substantial gainful activity.

Disability income (federal, state, and private disability benefits) is exempted from countable income calculations, and retirement arrangements under the Internal Revenue Code are exempted from resource limits.

Sliding-scale monthly premiums range from $20 minimum to $250 maximum based on countable income, with the Department of Human Services required to annually adjust premiums and report proposed changes to the legislature.

Implementation is contingent on the department determining that federal financial participation is available under Medicaid (Title XIX of the Social Security Act).
