Requires all health insurance policies, mutual benefit society plans, and health maintenance organization contracts issued or renewed after January 1, 2027 to cover biomarker testing for diagnosis, treatment, management, and monitoring of diseases or conditions.

Biomarker testing includes analysis of tissue, blood, or other biospecimens for gene mutations, protein expression, and whole genome sequencing when supported by FDA-approved indications, Medicare coverage determinations, or nationally recognized clinical practice guidelines.

Insurers must provide coverage in a manner that limits disruptions in care, including reducing the need for multiple biopsies.

If coverage is restricted, insurers must offer patients and providers a clear, readily accessible exception request process available on their website.

Extends the biomarker testing coverage requirement to Hawaii's Medicaid managed care program, with the Department of Human Services required to submit necessary state plan amendments to the Centers for Medicare and Medicaid Services.
