Establishes the Hanai Memory Network Program within the Executive Office on Aging to improve early detection, care coordination, and long-term support for individuals with dementia and their caregivers.

Creates dementia care specialists (trained social workers or allied health professionals) and memory clinics through partnerships with health care providers to provide community-based screening, diagnosis, and treatment services.

Requires the program to develop a statewide referral network, care options directory, and public-facing website serving as a central hub for dementia care education and provider listings.

Mandates annual reporting to the legislature on the number of individuals screened and diagnosed, care plans created, and provider/caregiver satisfaction data.

Appropriates unspecified general fund amounts for fiscal year 2026-2027 for program staff positions, equipment, clinical infrastructure, technology systems, and memory clinic partnerships, effective July 1, 2026.
