Adds a statutory definition for "juror qualification form" referencing the form described in section 612-13 of Hawaii Revised Statutes

Authorizes the court clerk or designee to determine the format of juror qualification forms and the method of delivery and return, removing the previous requirement that forms be mailed

Allows the court to contact prospective jurors more than once and by more than one method when distributing qualification forms

Prospective jurors must return completed forms within ten days of receipt, with penalties of up to $500 fine or 30 days imprisonment for willful misrepresentation of material facts
