Establishes the Hawaii Firearm Injury Restitution Fund, effective July 1, 2027, administered by the Department of Health to provide no-fault, first-payer coverage for medical care, mental health treatment, rehabilitation, lost wages, and funeral expenses resulting from firearm injuries.

Requires firearm manufacturers to obtain a Firearm Injury Cost Recovery License beginning January 1, 2027 as a condition of selling or distributing firearms in Hawaii.

Mandates manufacturer contributions to the fund based on risk categories determined by objective product characteristics and proportional share of statewide firearm injury costs, with rates subject to periodic actuarial review.

Specifies the fund does not impose civil liability on manufacturers, regulate firearm possession, or assign fault for criminal misuse, and must be consistent with federal law (Protection of Lawful Commerce in Arms Act).

Authorizes the Department of Health to deny, suspend, or revoke manufacturer licenses for noncompliance, with enforcement limited to licensing actions rather than civil or criminal penalties.
