Appropriates general fund revenues to increase Medicaid reimbursement rates for home and community-based residential services, including community care foster family homes and expanded adult residential care homes, to the "medium" rate scenario from Milliman's 2022 study.

Requires the Department of Human Services to obtain maximum federal matching funds and pursue all additional funding sources, including private grants, as a condition of expenditure.

Addresses findings that current direct care wages for home and community-based service providers fall below the 25th percentile nationally, and that a 2024 appropriation of $5,750,000 annual state share increase proved inadequate.

Responds to AARP research showing 75% of Hawaii residents aged 45+ consider affordable long-term care options in their communities an important independent living issue.

Takes effect July 1, 2050 (placeholder date), with the Department of Human Services responsible for administering the funds.
