Requires all individual and group health insurance policies, hospital service plan contracts, and health maintenance organization plans issued or renewed after December 31, 2025, to offer optional coverage for standard fertility preservation services

Coverage applies to insured persons undergoing medically necessary cancer-related treatments that may cause iatrogenic infertility (infertility resulting from surgery, chemotherapy, radiation, or other medical treatments)

Prohibits insurers from using prior diagnoses, prior fertility treatments, or discrimination based on expected length of life, disability, or perceived quality of life to restrict coverage availability

Standard fertility preservation services are defined by 2018 American Society of Clinical Oncology guidelines, but exclude experimental procedures, third-party assisted reproduction (donor eggs, surrogates), and cryopreservation storage

Effective date is December 31, 2050
